About the Role
We are seeking an experienced Community Psychiatric Nurse to provide high-quality care to patients in the community. You will be responsible for assessing patients referred to the CMHT due to mental health difficulties, identifying appropriate care pathways, and ensuring the least restrictive option to meet their needs. Additionally, you will signpost to care providers, attend Red Board meetings, and raise concerns to senior staff for support and management.
Key Responsibilities
- Providing robust assessments of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
- Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
- Signposting to appropriate care providers (including primary and tertiary care).
- Raising concerns to senior staff for support and management.
- Completing documentation and planning care for patients.
- Presentation cases to Multi-Disciplinary Team for discussion around care pathway.
- Liaison with colleagues nursing, occupational therapy, medics, psychology, and admin.
Requirements
To be successful in this role, you will need:
- A minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital environment within the last 3 years.
- Compliance with NMC Professional Registration and current nursing practices.
- A full UK driving license and own vehicle.

Salary and Location
The estimated salary for this role is £25-£35 per hour, depending on the location and trust. You will have the flexibility to choose from a range of Bank shifts across various Trusts in England.
