Job Overview

We are seeking an experienced and dedicated Community Psychiatric Nurse Specialist to join our team in Ealing Cognitive Impairment and Dementia Services, within the Older People's Mental Health Service.

This is a highly rewarding opportunity to work with individuals who present with cognitive impairment and dementia-related health needs, providing assessment, information, treatment, review, and support through individual and group work as part of a multi-disciplinary team.

As a key member of our team, you will work closely with individuals, their families, and carers to ensure the delivery of high-quality care and support. You will also contribute to the development and implementation of care plans, working collaboratively with other professionals to achieve the best possible outcomes for our patients.

Main Duties and Responsibilities
  • Provide specialist assessments, reviews, and nursing interventions to service users accessing the Cognitive Impairment & Dementia Service (CIDS).
  • Work collaboratively with multidisciplinary teams to deliver comprehensive care and support to individuals with cognitive impairment and dementia-related health needs.
  • Develop and implement effective care plans, ensuring the needs of individuals and their families are met.
  • Liaise with other professionals and organizations to ensure integrated and holistic care.
Required Skills and Qualifications
  • Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.
  • Evidence of continuous professional development.
  • Experience of working with patients and their carers with complex health and social care difficulties.
  • Supervisory experience, including managing clinical caseloads and leading multidisciplinary teams.
