We currently have a vacancy in South and West Devon, based at Follaton House Totnes, for either a full time or 2 part-time ( wte)team leader(s).

The successful candidate will need to be able to demonstrate the qualities in the person specification and job description and be committed to their on-going professional development to ensure quality in the delivery of care is maintained.

Main duties of the job

The team leader has responsibility for the day-to-day clinical and operational leadership and management of public health nursing teams in the Southern locality. This involves providing duty team leader cover as part of our PHN hub model (across whole locality with remote support to North locality specifically), and working collaboratively with local team leader colleagues and the wider Devon leadership team.

The team leader ensures the delivery of an effective, timely, and appropriate service, based on agreed priorities and performance indicators, which identifies need within families, schools and communities and addresses health inequalities. This includes first line safeguarding advice and supervision, identification and management of risk, and working in partnership with stakeholders and the wider community. In addition, team leaders support the development, implementation, and audit of service improvements and standards to ensure the delivery of needs-led evidence-based care that supports outcomes aligned to local and national priorities.

Job Purpose:

The post holder will have continuing responsibility for the day-to-day clinical and operational leadership and management of public health nursing (health visiting/school nursing teams) for a designated geographical area.

Ensures the delivery of an effective and appropriate service that identifies need within a family, school or community and addresses health inequalities. This includes first line safeguarding advice and supervision

To support the development and implementation of service development, standard setting, clinical audit and benchmarking, to deliver needs-led evidence based care that supports the local and national priorities.

Promote effective patient and public involvement in the development and delivery of services.

To be the principal point of contact for allocated GP practices, childrens centres, schools and the private and voluntary providers and facilitate partnership working. To develop and deliver co-operative working arrangements that support healthy outcomes for children, young people and their families

Ensures robust and accurate data recording systems are in place within the locality and that this is monitored regularly

To take a lead on specific public health priorities or project requirements across localities

Practice within the NMC code and Devon County Council policies and procedures2

This role requires the ability to fulfil all spoken aspects of the role with confidence and fluency in English.

Main duties and responsibilities:

Responsible for planning, developing and implementing systems and procedures that ensure that the public health nursing team delivers responsive care within the allocated resources.

Responsible for providing cover for other Public Health Nurse Team Managers as required

Responsible for working collaboratively with other Public health Nurse Team Managers and business support team leaders within a hub model to provide services across awider geographical area.

Responsible for the delivery of the national and local commissioned public health priorities breastfeeding & NCMP

Responsible for evidencing delivery of service priorities and outcomes and addressing challenges arising in relation to this.

Management and supervision of direct line reports (school nurses and health visitors) including initial safeguarding supervision and completion of appraisals.

Management of initial HR issues recruitment, sickness absence, performance and undertake investigations

Have financial oversight for the team budget and meet financial targets. This includes authorised signatory for overtime payments, bank staff, time sheets etc

Responsible for authorising expenditure and holds budgetary accountability for the procurement of physical assets and supplies community equipment.

Managing and developing the team to deliver the healthy child programme and meet key performance indicators.

Ensuring all team members have the competencies to undertake their role

Ensuring robust data returns and data validation

Engage with key stakeholders both internal and external and promote good working relationships

Uphold the organisations values and promote positive behaviours in the team and wider organisation

Patient Care Responsibilities:

To monitor and develop clinical practice and ensure clinical standards are evidence based and adhere to national and local guidance and policy

Completion of audits and implementation of action plans from audits, complaints etc

Support the development of quality initiatives and engage in the quality and governance agenda internal quality reviews, completion of annual quality account

Provides specialist clinical guidance to staff and other agencies in area of expertise

Participates in child protection case conferences, network meetings, and other multi agency meetings as appropriate and supports team members in the safeguarding process

Freedom to Act:

Acts as an autonomous public health nurse manager accountable for own professional actions.

Responsible for recognising the needs of the service, identifying risk and making autonomous decisions to provide appropriate management of the public health nursing team

Ensures that accurate and contemporaneous records are kept by staff in the team

Escalates risk appropriately to line manager, safeguarding supervisor

Location/Mobility
