About the Role

We are seeking a skilled Community Health Worker to join our Public Health Nursing team at Devon County Council. As a Community Health Worker, you will play a vital role in supporting families with children aged 0-8 years by working with them individually or as part of a group in both community and home settings.

Key Responsibilities
  • Support families with their child's development, including carrying out 1 and 2 1/2 year development reviews.
  • Integrate with the Health Visitors, School Nurses, and Staff Nurses to support with the whole family assessment.
  • Support parents and carers with health promotion and development topics such as behaviour management, toilet training, sleep management, and other targeted episodes of care as directed.
  • Communicate effectively with people who may have communication difficulties, including those whose first language is not English or those with sensory or learning difficulties.
  • Recognise situations where service users require advice and help from the Specialist Community Public Health Nurse, ensuring safe practice.
  • Use relevant equipment, such as electronic scales and measures of length and height, and interpret results.
  • Organise delegated workload to meet priorities of client care, making efficient use of time and travel.
  • Participate in supervision and annual appraisals, and use continuing professional development to consolidate, update, or improve clinical skills.
Requirements
  • QCF or equivalent qualification, NNEB / CACHE / BTEC Diploma (2 year full-time course), or NVQ 3 (Childcare related) Foundation degree or equivalent.
  • Evidence of continuing professional development.
  • Ability to work as a reflective practitioner.
  • Group and family work experience.
  • Child Protection awareness.
