About the Role

We are seeking a skilled and compassionate Community Health Worker to join our Public Health Nursing team at Devon County Council. As a Community Health Worker, you will play a vital role in providing preventative and supportive healthcare to children, young people, and their families.

Main Responsibilities

  • Provide preventative and supportive healthcare to children, young people, and their families
  • Work in partnership with families to provide advice and support
  • Signpost families to relevant services
  • Maintain accurate records of your work
  • Support and promote healthy lifestyles, good nutrition, and disease prevention
  • Communicate effectively with people who may have communication difficulties
  • Recognise situations where service users require advice and help from a Specialist Community Public Health Nurse
  • Use relevant equipment, such as electronic scales and measures of length and height
  • Organise delegated workload to meet priorities of client care
  • Participate in data collection, research, audit, surveys, and service user feedback

Person Specification

  • QCF or equivalent qualification in healthcare or a related field
  • Evidence of continuing professional development
  • Ability to work as a reflective practitioner
  • Group and family work experience
  • Child Protection awareness
  • Knowledge of services for children and families (desirable)
  • Experience in delivering health promotion (desirable)
  • Experience in assessing child development (desirable)
