Job summary

We have an exciting opportunity for an experienced ParamedicPractitioner to join our Primary Care Network (PCN). The post holder will bethe focal point for and work closely with Primary Care Network member practicesand clinicians to ensure that the Shropshire Rural Alliance PCN delivers thecontractual elements of the Network Direct Enhanced Service (DES) Contract

Main duties of the job

The successful candidate will ideally have primary careexperience and work within our practices as an autonomous, accountableParamedic providing holistic care for individuals including assessment,diagnosis, management, prescribing and treatment. The post holder will useadvanced clinical and communication skills to encourage and empower patients tolook after their own health and wellbeing and promote self-management whereappropriate.

Job responsibilities

This job description is only a very briefsummary of what a PCN Paramedic Practitioner job will entail, and the role willexpand and change over time as the needs of the PCN changes, the requirementsof the Network DES expands, and the priorities of the population evolve.

Primary Responsibilities

The following are the core responsibilities of the PCN ParamedicPractitioner. There may be, on occasion, a requirement to carry out othertasks. This will be dependent upon other factors such as workload and staffinglevels.

To holistically assess, diagnose, treat, orsignpost patients/service users with undifferentiated or undiagnosedconditions.

Clinically examine and assess patient needsfrom a physiological and psychological perspective, and plan clinical careaccordingly.

To complete our weekly Care Home rounds andclinically assess patients who have been prioritised for a review based on carehome advice and the MDT clinical judgement.

Complete annual reviews for patients who arehousebound and have chronic long term conditions, with the aim of increasingtheir confidence to self-manage and improve their health outcomes.

Review patients post-discharge who arehousebound, or temporarily housebound, and provide a person-centred holistichealth assessment of need.

Support our Home Visiting Team, if necessary,and provide clinical assessment and management of patients in their ownresidence.

To use advanced clinical skills to provideeducation to patients, promoting self-care and empower them to make informed choicesabout their own treatment.

Produce accurate, contemporaneous, andcomplete records of patient consultation, consistent with legislation, policies,and procedures.

Participate in setting and monitoringstandards to ensure effective care and delivery of evidence-based practice.

Work as an effective and responsible teammember, supporting others and exploring the mechanisms to develop new ways ofworking.

Ensure clear understanding and utilisation ofreferral mechanisms within their practice.

Prioritise own workload and ensure effectivetime-management strategies are embedded in own practice.

Demonstrate sensitive communication styles toensure patients are fully informed and consent to treatment.

If the post holder has completed anindependent prescribing qualification, they will be required to safelyprescribe for a range of common, urgent conditions in line with their personalformulary.

To identify the limitations of their ownpractice and liaise with senior colleagues where these limitations have beenreached to ensure good quality patient care is maintained.

Experience

Minimum of two years working as an advanced paramedic Awareness of telephone triage assessment Management of common minor injuries/illnesses Clinical examination skills Knowledge of needs of patients with long-term conditions Management of long term (chronic) conditions

Desirable

Clinical knowledge and understanding of the principles of evidence-based healthcare Experience of clinical governance including audit Experience of working in general practice
