The following are the core responsibilities of the PCN Care Coordinator. There may be, on occasion, a requirement to carry out other tasks. Report and communicate activity on a regular basis to the PCN Clinical Director and PCN Managers to ensure our on-going compliance with the Network Contract DES. Support in staff inductions, by coordinating IT training and following this up with appropriate check-ins when needed.
