Advanced Clinical Practitioner in Frailty

Band 8a - Secondment/Fixed Term (7 months)

The "Front Door" Frailty Practitioner will be part of the acute frailty team and working in the Emergency Department undertaking comprehensive holistic geriatric assessments for older patients living with frailty. They will be working as part of a new front door frailty team alongside another Band 8a Frailty practitioner, ED liaison physiotherapist and occupational therapist, with consultant geriatrician, pharmacy and social worker support and will be working in parallel with the ED department.

Main duties of the job

They will be providing early and appropriate assessments to identify those who need hospital admission and those whose needs may best be met by Same Day Emergency Care (SDEC) with or without linked primary care and community support. Succeeding in this should minimise patients' risk of exposure to COVID-19, along with other hospital associated risks such as deconditioning. This is particularly important for people aged over 65 years, who are more likely to present with symptoms of frailty. If treated as a priority, it is less likely these patients will need to be admitted to hospital.

The role will be responsible for the line management and professional leadership of the band 7 Frailty Practitioners within the integrated care networks.

The front door frailty team will aim to minimise and remove delays in the patient pathway, allowing services to care for patients within the same day, as an alternative to hospital admission.

Qualifications, Training

Essential

Registered Allied Health Professional/Registered Nurse Post registration experience gained by undertaking on-going personal development and training focused on older adults / frailty / advanced practice skills Prescriber (or working towards) Master's degree level award (or working towards) relevant to the role.

Desirable

Training in motivational coaching and/or interviewing

Skills and Abilities

Essential

Previous experience in completing advanced care plans. Able to lead a team successfully. Commitment to partnership working, including ability to work cooperatively with GPs, colleagues and other stakeholders. Strong communication skills (listening, verbal and written) that enable you to inform and advise others clearly, and communicate with a variety of audiences including clients, health and care professionals, commissioners and other organisations' staff. Ability to deliver services in a person-centred way (enabling/empowering approach) promoting independence in clients Knowledge of current community service changes and direction. Knowledge of clinical risk strategies and able to apply risk reduction techniques.

Experience of managing a caseload as a senior clinician. Demonstrable experience of working with people with health and social care needs, or community-based work with socially excluded groups, or those experiencing health inequalities. Successfully implementing change including change that involved staff. Experience managing staff within a team, resources and workload , effective use of team skill mix, appropriate delegation of patient care, supervision of junior staff.
