Senior Frailty Consultant Nurse B8B / B8C

We are pleased to announce the establishment of an Acute Frailty service at Croydon University Hospital, aimed at enhancing patient care.
We seek a Consultant Nurse/Practitioner to spearhead this initiative. This role is open to both Registered Nurses and Allied Health Professionals (AHPs) who possess a strong background in frailty management, clinical leadership, service enhancement, workforce development, and advanced practice. We also welcome applications from Trainee Consultant Nurses/Practitioners.
This newly created position is designed to implement and oversee an acute frailty service, primarily operating within the emergency care environment, while also engaging with acute wards to assess patient needs, deliver same-day services, and facilitate the effective utilization of alternatives to acute care, including Same Day Emergency Care (SDEC), virtual wards, and community rapid response services.
Key Responsibilities
The successful candidate will be expected to work independently to:
1. Establish a values-driven professional standard and evidence-based practices across various pathways, services, and organizations, collaborating with individuals, families, caregivers, communities, and other stakeholders. This will foster more integrated operations across the Acute Liaison and Frailty Assessment, Complex Care, and other teams within the OneCroydon alliance.
2. Provide leadership grounded in values across the care continuum, services, and systems, particularly in complex and evolving situations.
3. Cultivate staff potential, enhance workforce capabilities, and facilitate learning and development to promote excellence, encouraging interprofessional collaboration among diverse backgrounds, including nursing and AHPs.
4. Foster a culture rich in knowledge and inquiry throughout the service and system, contributing to research initiatives and positively impacting development, quality, innovation, and overall system effectiveness.
5. Establish expertise within the system and on a national scale by employing consultancy methods that significantly influence practice, services, communities, and populations, thereby enhancing workforce capacity and capability.

Essential
- Professional UK registration with NMC / HCPC and a minimum of 5 years of experience.
- A Master's level qualification in Advanced Clinical Practice or successful recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route, demonstrating educational equivalence at master's level (level 7).
- Management qualification or substantial senior management experience in a healthcare environment.
- Evidence of significant professional and managerial development, including a comprehensive and relevant Continuing Professional Development (CPD) portfolio.
Experience & Knowledge

- Demonstrated experience in establishing values-based professional expertise and evidence-based practices across pathways, services, and organizations within the Integrated Care Partnership (ICP).
