Position Overview

Senior Clinical Practitioner in Geriatric Care

Band 8a - Temporary Assignment

The Senior Clinical Practitioner will be an integral member of the acute geriatric care team, operating within the Emergency Department to conduct thorough and comprehensive assessments for elderly patients experiencing frailty. This role involves collaboration with a dedicated team, including another Band 8a practitioner, physiotherapists, occupational therapists, and support from geriatric consultants, pharmacists, and social workers, all while working closely with the Emergency Department.

Key Responsibilities

The practitioner will be tasked with delivering timely and suitable evaluations to determine which patients require hospital admission and which can be managed through Same Day Emergency Care (SDEC), potentially with the involvement of primary care and community resources. This approach aims to reduce the risk of exposure to hospital-related complications, particularly for individuals aged over 65, who are more susceptible to frailty symptoms. Prioritizing these patients can significantly decrease the likelihood of hospital admissions.

This position also includes the responsibility for overseeing and providing professional guidance to Band 7 practitioners within integrated care networks.

The geriatric care team is committed to eliminating delays in patient care pathways, facilitating same-day services as an alternative to hospital stays.

Qualifications and Training

Essential

  • Registered Allied Health Professional or Registered Nurse
  • Post-registration experience with a focus on ongoing professional development in geriatric care and advanced practice skills
  • Prescriber status (or actively pursuing)
  • Master's degree or equivalent qualification relevant to the role (or in progress)

Desirable

  • Training in motivational interviewing or coaching
