Are you a qualified social care practitioner registered with Social Work England? Join our dynamic team dedicated to supporting older adults in the Croydon area.
We are seeking a passionate and experienced social care practitioner to contribute to our essential mission – empowering individuals who require guidance, community resources, or direct assistance from the Council. Our approach is firmly rooted in a strengths-based model.
The leadership at Croydon Council is committed to supporting our statutory responsibilities, particularly in Adult Social Care and Health. We have clear priorities that guide our work.
Our team is well-established and stable, comprising social workers, health and wellbeing assessors, and a supportive team manager. We maintain low vacancy rates and prefer to focus on nurturing our permanent staff rather than relying on agency personnel. This includes having a dedicated training officer for adult social care and health, along with a Principal Social Worker to ensure staff welfare and professional development. We have also enhanced our electronic recording systems.
As a qualified Social Care Practitioner, you will be expected to have at least one year of post-qualification experience and be ready to take on the following responsibilities:
1. Assessment and Care Planning: Conduct thorough assessments of adults' needs, considering physical, psychological, and social aspects. Collaborate with service users, their families, and other professionals to create personalized care plans that are centered on the individual and promote independence.
2. Case Management: Oversee a varied caseload of adults with diverse needs, including older individuals, those with disabilities, and clients facing mental health challenges. Provide continuous support, advocacy, and service coordination to ensure individuals receive the necessary care to achieve their objectives.
3. Safeguarding: Promptly identify and address safeguarding issues in accordance with local policies. Collaborate with safeguarding partners, including health professionals and law enforcement, to ensure the safety and well-being of vulnerable adults.
4. Multi-Disciplinary Collaboration: Work effectively with colleagues from various fields, including health professionals, housing officers, and community organizations, to deliver comprehensive support to service users. Engage in multi-agency meetings and case conferences to coordinate care and foster collaborative practices.
5. Promotion of Independence: Assist adults in maintaining or regaining their independence by connecting them with community resources, social activities, and housing options. Provide practical help with tasks such as budgeting, housing applications, and accessing benefits to encourage self-sufficiency and empowerment.
6. Record Keeping and Reporting: Keep accurate and current records of assessments, care plans, and interventions using electronic systems. Prepare clear and concise reports for case conferences, court hearings, and other formal reviews, ensuring compliance with legal and regulatory standards. You will also participate in the duty rota within the team.
7. Duty Rota Participation: Engage in the Locality duty rota.
This position is part of the Croydon Council's Locality Team, where you will collaborate with other professionals in an integrated service to conduct strengths-based assessments that promote independence and support individuals in living in their own homes.
Requirements: Must possess a full British driving license and be registered with Social Work England.
