Senior Social Care Practitioner – Purley Locality Team

Grade 10 – (£40,833 - £42,840)

We are seeking a dedicated and experienced social care practitioner to contribute to our essential mission – empowering individuals who require guidance, community assistance, or direct support from the Council. Our approach is firmly rooted in a strengths-based model.

The leadership at the Council has expressed strong support for our statutory responsibilities, particularly in Adult Social Care and Health. We are clear about our priorities as a council.

Our team comprises stable and experienced social care practitioners, health and wellbeing assessors, and a supportive team manager. We maintain low vacancy rates and prefer to invest in our permanent staff rather than relying on agency personnel. This commitment includes a dedicated training officer for adult social care and health, along with a Principal Social Worker focused on staff welfare and professional development. We have also recently enhanced our electronic record-keeping system.

As a qualified social care practitioner, you are expected to possess at least one year of post-qualification experience and be ready to undertake the following responsibilities:

1. Assessment and Care Planning: Conduct thorough assessments of adults' needs, considering physical, psychological, and social factors. Collaborate with service users, their families, and other professionals to develop personalized care plans that prioritize person-centered interventions and promote independence.

2. Case Management: Manage a varied caseload of adults with diverse needs, including older individuals, those with disabilities, and individuals facing mental health challenges. Provide continuous support, advocacy, and service coordination to ensure individuals receive the necessary care and assistance to achieve their objectives.

3. Safeguarding: Promptly identify and address safeguarding concerns in accordance with local policies and procedures. Collaborate with safeguarding partners, including health professionals and law enforcement, to ensure the safety and well-being of vulnerable adults.

4. Multi-Disciplinary Collaboration: Work effectively with colleagues from various disciplines, including health professionals, housing officers, and voluntary organizations, to deliver comprehensive support to service users. Engage in multi-agency meetings and case conferences to coordinate care and facilitate collaborative practices.

5. Promotion of Independence: Assist adults in maintaining or regaining their independence by connecting them with community resources, social activities, and housing options. Provide practical support with tasks such as budgeting, housing applications, and accessing benefits to foster self-sufficiency and empowerment.

6. Record Keeping and Reporting: Ensure accurate and timely documentation of assessments, care plans, and interventions using electronic systems. Prepare clear and concise reports for case conferences, court hearings, and other formal reviews, ensuring compliance with legal and regulatory standards. You will also participate in the duty rota within the team.

7. Duty Rota Participation: Engage in the Locality duty rota.

There is one available position within the Purley Locality Team, based in Bernard Weatherill House. You will collaborate with other professionals in an integrated service, conducting strengths-based assessments to promote independence and support individuals in remaining in their homes.

Requirements: Must possess a full British Driver's License and be registered with Social Work England.
