We are seeking a highly skilled and experienced Consultant Practitioner to lead the development and implementation of our Acute Frailty Service at Croydon University Hospital.
Key Responsibilities- Establish and lead a values-based professional expert and evidence-based practice across pathways, services, and organisations within the Integrated Care Partnership (ICP).
- Develop and implement service development strategies to ensure the Acute Frailty Service meets the current population health needs.
- Lead change at a local and national level, aligning with Government agendas and driving organisational change.
- Provide leadership and expertise to the multidisciplinary team, promoting excellence and driving high-quality care.
- Develop and maintain a 'knowledge-rich and inquiry' culture across the service and system, contributing to research outputs and driving innovation.

Requirements- Professional UK registration with the NMC or HCPC.
- Master's level qualification in Advanced Clinical Practice or equivalent.
- Management qualification or significant senior management experience in a healthcare setting.
- Evidence of significant professional and managerial development, including a comprehensive and relevant CPD portfolio.
- Experience of establishing values-based professional expert and evidence-based practice across pathways, services, and organisations within the ICP.
- Experience of service development and applying continuous quality improvement methodology.
- Ability to demonstrate a sound understanding of health policy and apply that knowledge to the ICP for the benefit of the population.
- Highly advanced planning, organisational, leadership, and communication skills.
