Frailty Practitioner (Allied Health Professional/Nurse)
Fixed Term / Secondment for 6 months
Band 7 - full-time hours
The team will be integral in ensuring patients receive integrated care across acute frailty pathways, relevant departments and services, and the community enabling teams to work more collaboratively, resulting in patient care being less fragmented or delayed as well as being a more positive experience with better outcomesActively provide multifactorial assessment and intervention to access appropriate rehabilitation pathways to patients to reduce unnecessary hospital admission, and provide early supported discharge.
To manage the discharge of patients with complex needs, using evidence based/patient centred principles to support the implementation of interventions in their own home, or in a rehabilitation bed.
To provide line management for junior staff through supervision; creating opportunities to enhance knowledge, skills and behaviours that help to build competencies and undertake appraisal.
To provide a coordinated approach through established work streams on a daily basis to support standard operating procedures.
To participate in maintaining an overview of the standards of care being delivered, instigating remedial action to improve practice as necessary.Interviews: tbc
Main duties of the job
To work with a high level of autonomy, within agreed guidelines, managing undifferentiated and undiagnosed problems using advanced clinical skills such as physical assessment, diagnosis and negotiate with patients and carers to develop safe discharge plans to meet the assessed needs that balance risk in an efficient and sensitive way, offering choice were possible. This may include step up beds, packages of care or use of other community demonstrate and supply equipment and ensure patients, families, carers and support workers are confident in the safe and correct use of equipment at the time of issue, with regards to the services ensure the team delivers clinically effective care and health promotion that is evidence basedTo communicate effectively with patients and / or carers appropriate complex information regarding the patient's medical condition and inform them of the benefits and risks of suggested treatment work collaboratively and communicate effectively with a wide range of colleagues, coordinating a patient focused management approach to facilitate timely discharges. The patient group includes those patients who are frail and at high risk of unplanned hospital admissionTo keep accurate contemporaneous written and/or electronic documentation, care plans and reports
