The successful candidates will work within the locality health visiting team across Plymouth. the competency-based framework and learning opportunities will support your access to the Specialist Public Health Nursing Programme (SCPHN/HV). this post will also be supported by the locality managers, colleagues, a named supervisor and practice assessor, to ensure that all learning outcomes and competencies are completed prior to commencement onto the SCPHN HV course.

The specialist public health nursing programme is offered by Livewell Southwest at UWE.

All Livewell Southwest staff are expected to able andwilling to work across a 7 day service.

Main duties of the job

The Student Specialist Community Public Health Nurse is a registered nurse/midwife undertaking a full time training course consisting of practice and theory components. The course will require attendance at UWE for academic modules and clinical learning at practice placements within Livewell Southwest

On successful completion of the course the post holder will be eligible to register with the Nursing and Midwifery Council as a specialist community public health nurse.

The post holder will be required to undertake study and duties to gain the appropriate knowledge and skills to ensure high quality, evidence based assessment, delivery and evaluation of care in relation to child/family need.

During practice placements the post holder will work under the supervision of a mentor and practice teacher, as part of the public health nursing team in providing preventative and supportive health care to children, young people and their families. As part of the public health nursing team the post holder will work collaboratively with a wider multi-disciplinary and inter-agency team in offering public health services to the community. The work takes place in a variety of settings, including home, clinic and community venues.

Job responsibilities

Under the supervision of the practice mentor/teacher, participates in the assessment of health needs and formulation of care plans for children, young people, their families and communities, this includes the implementation of programmes of care, offering support and advice to parents and families in the home, clinic, school or community setting parenting skills, public health, group work and child health clinics.

In collaboration with the public health nursing team, identifies individuals and groups within the community where collaborative working and advice would forward the interests of public health and promote healthy living, in particular seeking to identify disadvantaged groups and address health inequity.

Offers advice and support to children, young people and adults in accordance with evidence based practice.

Safely delivers immunisations in line with policy and patient group directions.

Under the supervision of the Practice Mentor/Teacher completes the Common Assessment Framework, working with the multi-agency team in supporting children in need and those at risk of significant harm, including observing in case conferences and other meetings and contributing to reports where appropriate. This includes learning about the role of lead professional or key worker for children and their families with the most complex needs.
