Job summary

The Norwich Triage team currently has a vacancy for a Senior Triage Clinician.

The Triage Hub is the starting point of the patient journey into community services, we take great pride in our work and always aim to do the best for our patients. We are looking for someone who holds the same values and wants to be part of a diverse and supportive team.

We are a small and friendly team of nurses who triage patient referrals from across the Norwich area. The Senior Triage Clinician is a pivotal role within the Triage Hub. The Senior Triage Clinician (STC) has oversight of the day to day, co-ordination of patient visits across Norwich. The STC role is a varied role which includesworking responsively to changing demands across the community teams to ensure patients are prioritised effectively and staff are supported, providing guidance to Triage nurses and carrying out complex triage.

Although the role is predominantly office based working in the Triage hub, the role does allow for clinical time which can be spent seeing patients in their own homes or in clinic environments. We are keen to help grow and develop our staff, with many training and learning opportunities available for those working at NCH&C.

Main duties of the job

Offer advice and support to other members of the primary healthcare team. Provide a mentorship role to HUB staff. Be involved in the provision of relevant professional advice and expertise in matters regarding the service. Have oversight of the day to day, co-ordination of patient visits across Norwich. Keep accurate, confidential and up to date records for all patient contacts. Ensure activity data and clinical information is recorded in the patient held record (where available) and on SystmOne and completed on a daily basis. Be familiar with and comply with all relevant measures under the data protection act and Caldecott guardian initiative, vulnerable adult policies (including safeguarding) and the NCH&C clinical governance framework. Attend regular supervision meetings with a nominated supervisor and provide supervision for other team members. Participate in continuing professional development activity and all mandatory training as required. Practice within NMC / COT / CSP professional codes of conduct and protocols. Promote the effective use of local services by closer networking.

Maintain comprehensive and accurate electronic documentation in order to demonstrate a robust clinical decision-making audit trail for themselves and/or the team. Remain accountable for own professional actions as determined by the professional bodies. Provide advice/training and supervision to triage staff including PDP's Follow Trust policies, and local procedures, and use discretion in interpreting them according to the setting and circumstances. Maintain own continued professional development. Be accountable for work delegated to others. Attend in-house training as required to develop current job role.

Job responsibilities

Lead staff in ensuring the timely processing of referrals by evaluating the information received, often complex and sensitive, to enable the allocation of the referral to the most appropriate service. Use advanced clinical knowledge to prioritise the most appropriate response required. Liaise with referrers ( GPs, social services, acute sector) and patients to inform the decision making process when deciding the appropriateness and urgency of a referrals. To include using advance communication skills and interpretation skills can you specify what type it is tact, persuasive; are there barriers to communications. Manage the expectation of referrers and patients when services are unable to respond as expected. To include using tact and diplomacy to deal with verbal aggression. Develop and implement a triage rota to ensure the effective use of resources whilst ensuring that the triage service is staffed appropriately to meet varying demands throughout the day. To include monitoring and analysis of statistics to determine the busiest times of the day/days of the week and adopting a flexible approach, re-prioritising work accordingly for the triage staff raising any concern regarding lack of resources to the Clinical Operational Manager. Use advance judgement skills to determine and re-prioritise own workload and that of other colleagues to meet frequently fluctuating demand. Make recommendations for service improvement to Clinical Operational Manager based on own experience and feedback from staff and service users. Lead on the implementation of new processes and procedures within the triage team, using negotiating and engagement skills to ensure compliance by staff. Person Specification
