Harpenden Health PCN are looking for an enthusiastic Care Coordinator Administrator. A part-time permanent role working between 18-22 hours per week with flexibility around school hours (if applicable).
Care Co-ordination requires a dedicated self-starter, enthusiastic to work with 3x GP Practices atscale. The successful candidate willbe based in a local cluster of Practices as part of the PCN. We are looking for a caring, dedicated,reliable and person-focused individual, who enjoys working with a wide range ofpeople on a variety of clinical areas. You will need to have good organisational skills, confident in IT / MS Office packagesand possess excellent administration skills.
Main duties of the job
This role is Non-Clinical.
Care coordinators work closely with the PCN clinical teams todeliver demanding National vaccination programmes. The successful candidate will be liaisingverbally with patients who have complex health needs. In addition to liaising with Patients, youwill also be liaising with the local voluntary sector. We are looking for someone highly motivated, proactivewith a flexible attitude. Job responsibilities
Coordination and Integration
Establish strong working relationships with GPs and practice teamsand work collaboratively with other Care Co-ordinators, Social Prescribing LinkWorkers and Health and Wellbeing Coaches, supporting each other, respectingeach others views, and meeting regularly as ateam.
Act as a champion for personalised care and shared decision makingwithin the PCN; assisting in monthlyMDT meetings, establishing, and maintaining links with our Care Homes.
Supporting the work of the clinical leads of the PCN in deliveringa holistic and timely support in relation to the NHS Enhanced Service andpriorities of the wider NHS England.
Liaising with our Clinical Pharmacist teams to ensure patientsreceive timely and appropriate medication reviews.
Ensuring regular and effective communication with our localsupport agencies within and outside the NHS.
Demonstrate a flexible attitude and be prepared to carry out otherduties as may be reasonably required from time to time within the generalcharacter of the post or the level of responsibility of the role, ensuring thatwork is delivered in a timely and effective manner.
Undertake continual personal and professional development, takingan active part in reviewing and developing the role and responsibilities, andprovide evidence of learning activity as required.
Care and support
Have apositive, empathetic, and responsive conversation with the person and theirfamily and carer(s) about their needs.
Support patientsto develop and implement personalised care and support plans. Ensurepersonalised care and support plans are communicated to the GP and any otherprofessionals involved in the patients careand uploaded to the relevant online care records, with activity recordedusing the relevant SNOMED codes.
Key Areas
Help patientsto manage their needs through answering queries on the telephone, making andmanaging appointments, and help them make choices about their care.
Providecoordination and navigation for patients and their carers across health andcare services, working closely with Social Prescribing Link Workers, Health andWellbeing Coaches, and other primary care professionals; helping to ensure patients receive a joined-upservice and the most appropriate support.
Workcollaboratively with GPs and other primary care professionals within the PCN.
Support the coordination anddelivery of multidisciplinary teams with thePCN.
Support PCNs;developing communication channels with GPs, patients, their families, carers,and agencies.
Conductfollow-ups on communications from out of hospital and in-patient services.
Supportpractices to keep care records up to date by identifying and updating missingor out-of-date information about the personscircumstances.
