Job summary

An exciting opportunity has arisen to join our passionate and forward-thinking frailty team and virtual ward. A role has become available on a fixed term basis for 12 months as an Advanced Clinical Practitioner. Working with the current team of Consultants, ACP's, GP trainees and specialist nurse, you will play a pivotal role in the identification, assessment and management of patients who would benefit from the collaborative care of the virtual ward.

You will also have a regular opportunity to work across the 4 pillars of advanced practice. Your involvement in education would be encouraged, along with audit, service development and leadership.

We welcome applicants to apply for both an ACP post following completion of MSc, and trainee ACP who are currently on the MSc pathway.

Main duties of the job

Practice autonomously as an ACP using collaborative and independent judgement to assess, investigate, diagnose, plan, implement, manage, and evaluate clinical nursing and medical care in the management of patients

You will be responsible for the initial treatment of patients presenting with undifferentiated or undiagnosed conditions and make professional autonomous decisions regarding treatment within your scope of practice

Prescribe medicines independently and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within scope of practice to optimise treatment.

Liaise with other members of the multidisciplinary team to plan the safe discharge of patients when required

Liaise with nursing staff, Consultants and junior doctors on an on-going basis, informing them of patients management plans as appropriate

Plan and co-ordinate the patient's assessment, treatment and discharge/admission

Participate in audit, clinical governance and other clinical meetings where the outcomes and recommendations have the potential to improve the quality of care; give case presentations as required.

Contribute to the development and implementation of clinical policies, procedures and guidelines

STAFF CAR PARKING

Staff Car Parking on the West Suffolk Hospital Site is eligible for individuals working outside of the core hours, 07:00 18:30.

For individuals working within the core hours, the Trust offers parking off site at the Bury St Edmunds Rugby Club, which is serviced by a regular shuttle bus to and from the Hospital. Details of the bus timetable can be found in the on-line starter pack on the West Suffolk Hospital website.

This parking restriction does not apply if an individual is required to work on a weekend or if they are required to complete on call duties within their role.

Education and qualifications

Essential

Registered nurse or equivalent AHP First or second-class honours degree (minimum 2:2) from a UK university or comparable institution Pg Dip Advanced Nursing Practice or equivalent (level 7). MsC Advanced clinical practice Recognised, up to date Non-medical prescribing qualification Evidence of continuous professional development Postgraduate ENB 998 Teaching / Mentorship Course or equivalent

Desirable

ILS/ALS Further professional qualification ANP/ ACP linked to role Qualification in advanced communication skills

Experience and knowledge

Essential

Extensive recent relevant experience of face-to-face patient care Extensive experience of working as a trainee in advanced practice at band 7 or above Demonstrates evidence of clinical competencies in acute emergency care Experience of undertaking expanded roles within the speciality applied for

Desirable

Demonstrable experience and involvement in clinical audit methods and quality initiatives Experience of teaching
