An exciting opportunity has arisen to join our passionate and forward-thinking frailty team and virtual ward. A role has become available as an Advanced Clinical Practitioner, as we continue to develop our frailty virtual ward pathway. Working with the current team of Consultants, ACP’s, GP trainees and specialist nurse, you will play a pivotal role in the identification, assessment and management of patients who would benefit from the collaborative care of the virtual ward and frailty team.

You will also have a regular opportunity to work across the 4 pillars of advanced practice. Your involvement in education would be encouraged, along with audit, service development and leadership.

**JOB PURPOSE**:
To practice as an Advanced Clinical Practitioner (ACP) within own sphere of competence and agreed acceptable limits of practice

Responsible for assessing, diagnosing, interpreting investigations and treating patients.

To work as part of the multi-disciplinary team in providing quality evidence-based health care to meet the immediate needs of the patient

To be responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care, without supervision, and to teach other nursing and non-nursing staff

To provide day-to-day clinical leadership to the junior doctor and nursing teams, participating fully in the delivery of healthcare provision.

Apprentice Responsibilities (if applicable)

Attend all off-the-job training components of the apprenticeship as agreed with your line manager and apprenticeship training provider

Proactively engage in on the job activities in the work place to become competent in the apprenticeship standard and role.

Complete all apprenticeship course work, assessments and self-directed study in a timely way as agreed with your workplace supervisor and apprenticeship training provider.

Undertake all statutory and mandatory training required for the job role. This will be in addition to the apprenticeship training requirements.

Practice autonomously as an ACP using collaborative and independent judgement to assess, investigate, diagnose, plan, implement, manage, and evaluate clinical nursing and medical care in the management of patients.

You will be responsible for the initial treatment of patients presenting with undifferentiated or undiagnosed conditions and make professional autonomous decisions regarding treatment within your scope of practice

Prescribe medicines independently and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within scope of practice to optimise treatment.

Liaise with other members of the multidisciplinary team to plan the safe discharge of patients when required

Liaise with nursing staff, Consultants and junior doctors on an on-going basis, informing them of patients management plans as appropriate

Plan and co-ordinate the patient’s assessment, treatment and discharge/admission

Participate in audit, clinical governance and other clinical meetings where the outcomes and recommendations have the potential to improve the quality of care; give case presentations as required.

Contribute to the development and implementation of clinical policies, procedures and guidelines

Promote the delivery of evidence-based care to all patients, recognising the limitations of the evidence allowing autonomous decisions to be made

Teach and support medical staff, student and newly qualified/
