About us

Ivry Street Medical Practice is based just outside of the town centre, near to the popular Christchurch Park. We have four partners and 4 salaried GPs, along with existing Nurses, ANP, Paramedic, HCAs, Social Prescribers and Mental Health teams, along with reception and administrative teams to provide support, we total 49 staff members currently

The practice holds a list size of 13500 patients.

Job description

Job responsibilities

Primary Responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. To support the Infection Prevention Control (IPC) lead and to work to the IPC policy, supporting the implementation of audits, training, support and evidence of compliance to the Practice Manager and IPC lead as required

b. To support the production of the IPC annual statement in conjunction with the IPC lead

c. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

d. To develop, implement and embed health promotion and wellbeing programmes (usually as part of a review and referral discussion with the patient)

e. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

f. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients' health

g. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and NSF

h. As required, to manage acute chronic conditions

i. To prescribe (if training permits) for patients in accordance with PGDs and own scope of practice

j. To review patients' medications to enhance compliance

k. To provide guidance to patients in the use of prescribed medications regarding side effects and doseages

l. To coordinate the provision of travel medications / services

m. To request pathology services and process pathology results as required (in accordance with own scope of practice)

n. To be responsible for ensuring own specimens are recorded and ready for onward transportation

o. Ensure that any fridges in the room you work from are cleaned routinely in accordance with extant guidance

p. Ensure your clinical working room is stocked and prepared for each session

q. To maintain accurate clinical records in conjunction with extant legislation

r. To ensure read codes are used effectively by all members of the nursing team

s. To adhere to practice policies and protocols for clinical staff

t. Ensure mandatory training is kept up to date

u. To ensure compliance with the practice chaperone policy

v. To support the wider team in dealing with clinical emergencies

w. To implement vaccination programmes for adults and children

x. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc)

y. To support the clinical team with all safeguarding matters, in accordance with local and national policies

z. To deliver opportunistic health promotion where appropriate

Secondary Responsibilities

In addition to the primary responsibilities, the Healthcare Assistant may be requested to:

a. Implement nursing protocols in conjunction with the Nursing Team Lead, Practice Manager and partners

b. Participate in local initiatives to enhance service delivery and patient care

c. Support and participate in shared learning within the practice in order to improve patient care this can include observations from other clinical / non-clinical staff

d. Continually review clinical practices, responding to national policies and initiatives where appropriate

e. Participate in the review of significant and near-miss events applying a structured approach, , root cause analysis (RCA)

Qualified in phlebotomy Experience to help us manage QOF
