Lead Practice Nurse

Martlesham Heath Surgery is a dynamic and patient-focused healthcare organization seeking an experienced and skilled Lead Practice Nurse to join our clinical team.
Key Responsibilities- Manage and oversee the practice nursing team, ensuring effective delivery of clinical services to patients.
- Develop and implement evidence-based care management programs for patients with long-term conditions, including elderly and housebound patients.
- Act as the Infection Prevention Control (IPC) lead, ensuring compliance with IPC policy and providing training and support to the team.
- Develop and implement health promotion and wellbeing programs, promoting healthy lifestyles and disease prevention.
- Prescribe medication for patients in accordance with PGDs and own scope of practice, and review patient medications to enhance compliance.
- Provide routine nursing care to patients in accordance with clinical-based evidence, NICE, and the NSF.
- Collaborate with the practice management team to review and deliver clinical policy and procedure.
- Support the team in dealing with clinical emergencies and provide guidance on clinical procedures.
- Recognize, assess, and refer patients presenting with mental health needs, and understand practice and local policies for substance abuse and addictive behavior.
- Registered Nurse (Nursing and Midwifery Council)
- Postgraduate diploma or degree in Chronic Disease Management
- Nurse prescriber
- Completed formal infection prevention and control training
- Family planning qualification
- Experience of working in a primary care environment, leading a nursing team, and managing chronic disease
- Excellent communication and interpersonal skills, with the ability to work autonomously and as part of a team
