Main duties of the job
Co-ordinate care for residents of care homes, ensuring their needs are addressed.

Work closely with GP practices and the Primary Care Network (PCN).

Job description
Job responsibilities
Identify and manage a caseload of patients in need of care packages.
Engage with patients, carers, and advocates to develop holistic care plans.
Provide guidance on managing health and accessing support services.
Liaise with local health and care services to co-ordinate care.
Maintain up-to-date records and ensure all care plans are followed.

Person Specification
Qualifications
Essential
Good general standard of education. Experience working with people to empower them and improve their wellbeing. Strong communication and interpersonal skills. Ability to work on own initiative and manage time effectively. Knowledge of healthcare systems, safeguarding, and confidentiality.
Desirable
Health and social care qualification. Experience in a multidisciplinary team. Familiarity with issues related to dementia and complex health needs. Use of IT systems including EMIS and Microsoft Office.
