Job summary

We are looking for an experienced and versatile nurse to work primarily within our Hospice at Home Service as well as, to support all clinical areas on a day to day basis.

You will work closely with our Enhanced Support Service and ensure timely pathways for patients supporting discharge from hospital.

Our Hospice at Home service brings the skills and expertise of our highly trained and dedicated specialist team directly into our patients homes. This allows patients, their families, and carers access to the vital support they need at home, when time matters most.

Main duties of the job

The post holder works autonomously within defined protocols to facilitate and provide high quality specialist nursing care to patients with palliative care needs for Princess Alice Hospice. The post holder will act as a specialist resource for other health and social care professionals on issues relating to specialist palliative care, the community and respond to their identified requirements. The post holder will support all clinical areas on a day to day basis acting as liaison between Triage, Enhanced Support Service and the wider Hospice at Home team.The post holder will respond to urgent requests for support from patients and families in crisis in order to avoid inappropriate admissions to hospital.

Job responsibilities

Clinical:

In collaboration with other members of the palliative care multi-professionalteam assess the needs of patients, families and other carers and providespecific advice and support as appropriate.

Meet with other members of the MPT to ensure that acomprehensive service is offered to all patients.

Collaborate with other health care professionals in thecommunity in order to promote planned, coordinated care. Support the community team manager, Nurse Consultant and other colleagues byworking flexibly, being able to cover a caseload during annual leave andsickness, in order to ensure consistent care is able to be delivered. Ensure patients are supported using a multi - professional approach and arereferred in a timely manner according to multi professional guidelines forbest practice. Maintain accurate records, ensuring that all entries on Crosscare and relateddocumentation are entered frequently to agreed standards. Liaise with other agencies to facilitate systems for transfer of information asnecessary. Working within the defined Rapid Response protocol and in collaboration withTriage and the CNSs support the Rapid Response /Urgent care element of theHospice at Home Service, visiting patients and families in crisis in order toprevent inappropriate admissions to Hospital. Participate in the on call Rota which covers Princess Alice Hospice catchmentarea.

Function as a professional role model for specialist palliative care, providingstrong and effective clinical leadership. Undertake complex and sensitive holistic care assessments using physicalassessment skills to plan, implement and evaluate care, utilising care planningand excellent communication skills. Improve access to medication through the support and implementation of NurseIndependent Prescribing

- Prescribe within scope of practice / area ofcompetency

- Follow locally approved clinical guidelines andNICE and be within an agreed formulary if applicable.

- Provide cost effective prescribing activity.

- Regularly review prescribing activity andimplement change as necessary.

- Have prescribing activity audited annually.

- Work within the framework of the HospiceNon-Medical Prescribing Policy.

Management:

Support the locality leads and Associate Director in delivering the strategicdirection of the Hospice and the clinical business plan.

Submit statistics and activity reports as required. Influence palliative care nursing practice and policy through working inpartnership and networking with colleagues. Ensure the effective use of resources required to meet service objectives.Promote harmonious relationships within the inter-professional team. Act as an ambassador for Princess Alice Hospice at all times, includingrepresenting the Hospice in fundraising and PR activities as requested. Receive and disseminate information through attendance at team meetings, themonthly H@H team meeting and appropriate meetings with other hospices andpalliative care teams.

Educational:

Continue personal professional development, to ensureclinical effectiveness of self and to have a positive commitment to ensure thatall appraisal objectives are met.

Feedback all materials and information obtained at study days and other formsof study. Receive regular clinical supervision to ensure the maintenance of professionalgrowth. Participate in the development and delivery of formal educational programmesfor professionals in Princess Alice Hospice, the community and othereducational establishments. Act as role model and mentor, providing informal education and support tocolleagues in their provision of palliative care. Identify the specific training and educational needs of patients and carers andfacilitate meeting these needs.

Audit & Research:

Provide leadership and support to others in the development of research-basedpractice. Develop mechanisms for auditing care and in conjunction with senior medical andnursing staff, participate in the development of multi professional audit. Participate in and support collaborative clinical research in palliative care. Critique current research into best practice and promote and manage changewithin the community palliative care team and other relevant organisations andagencies. Ensure that the views and needs of both lay and professional users of theservice influence its ongoing development by feeding back as appropriate.
