Job OverviewWe are seeking an experienced and versatile nurse to join our Hospice at Home service as a Band 6 specialist. This role involves working closely with our Enhanced Support Service to ensure timely pathways for patients supporting discharge from hospital. You will respond to referrals where an urgent visit is required to support patients and families in crisis, enabling more patients to achieve their preferred place of care/death.
Detailed Job DescriptionThis is a challenging yet rewarding opportunity for a skilled nurse to develop into the role of a Rapid Response CNS. The successful candidate will work primarily within our Hospice at Home Service, supporting all clinical areas on a day-to-day basis. Key responsibilities include:
- Delivering high-quality patient care through effective communication and coordination with multidisciplinary teams
- Supporting timely discharges from hospital by facilitating seamless transitions to community-based care
- Responding to referrals and providing urgent visits to support patients and families in crisis
