We are looking for a caring and compassionate individual who is passionate about working with young people with eating disorder and their families to join our team.

Hospital at Home is an exciting innovative service developed in collaboration with the Thames Valley Provider Collaborative. The Hospital at Home team remotely supports young people with eating disorders who meet the threshold for inpatient care. In some cases, admission will be prevented and in others, young people will be stepped down to this model following a short inpatient admission. Hospital at Home maximises the use of new technology to virtually provide care in the patients home, reduce admission, length of stay, waiting times and provide earlier discharge from inpatient care.

The purpose of the Nurse Associate will be: 

· To ensure that a responsive high quality, safe and patient focused service is delivered.

· To work effectively and in partnership with key members of the Multidisciplinary Team to promote and facilitate effective patient care and discharge.

· To develop and sustain close and effective working relationships with partner services.

· To support healthcare support worker colleagues in their professional development and in the promotion of best practice.

Main duties of the job

DUTIES AND RESPONSIBILITIES

· To support the healthcare support workers in their day-to-day work and promote the concept of interdisciplinary team working.

· Contribute to expert assessment of patients need and risk under supervision of Registered Nurse.

· To promote optimal levels of independence and wellbeing for patients.

· To be accountable for an identified caseload.

· To assess, plan and evaluate individual treatment and risk management programmes of care in partnership with patients, carers and other health and social care professionals in a response to planned and urgent need.

· To ensure understanding of conditions and treatment and to have relevant knowledge of physical health and signs of deterioration.

· To complete accurate documentation of all relevant interactions in line with professional body/ guidance

· Attend the multidisciplinary team meeting.

· Care planning, risk assessment and management, discharge planning.

Detailed job description and main responsibilities

· Deliver and evaluate specialized individual programmes of care

· Advising, where appropriate, on changes to treatment and management

· To assess, plan and evaluate individual treatment and risk management programmes of care in partnership with patients, carers and other health and social care professionals in a response to planned and urgent need.

· To communicate effectively with clients /carers and colleagues to agree treatment goals.

· To ensure understanding of conditions and treatment.

· To deliver a range of clinical skills which may include, for example, physical examination, ordering, performing and interpreting common screening and diagnostic tests.

· To utilise clinical expertise in identifying patients’ needs specifically around the early signs of deterioration to ensure acute changes in condition are responded to appropriately

· To complete accurate documentation of all relevant interactions in line with professional body/ guidance

· Ability to advocate for patients and others

· To facilitate the Multi Disciplinary Team meeting and ensure key issues are shared across the MDT.

· Responsibility for the monitoring of efficient and effective use of resources.

· Ensure patient safety and Safeguarding policies and procedures are adhered to.
