Job summary

Humbleyard Practice is seeking a clinical pharmacist on a full-time basis for hours per week, to improve the quality of patient care across our surgeries.

You will possess motivation and passion to deliver excellent service by undertaking clinical medication reviews to proactively manage patients and provide support to general practice staff, patients, and their carers in regard to medication & prescription queries.

Main duties of the job

Act as main contact for all medicine related queries, providing specialist medicines information expertise & advice in the use of medicines. Provide leadership on person-centred medicines optimisation and quality improvement. Provide structured medication reviews & health education advice to patients through face-to-face, telephone or notes-based reviews. Assist in the care & monitoring of patients with complex, long-term conditions. Capture relevant patient health information at all consultations. Support improvements in clinical care through audits & implementing agreed quality improvements. Process specialist treatment & hospital discharge letters where medicines optimisation or reconciliation is required. Manage the repeat prescribing process, through reviewing requests & medicines reaching review dates.

Humbleyard Practice is a 22,000-patient part-dispensing three-surgery practice conveniently located near to the fine city of Norwich and close to the major road connections for the county. We are a teaching practice with medical, PA and nursing auxiliary students and keen to further develop our existing staff.

We are part of the five-practice Ketts Oak Primary Care Network with a strong PCN clinical pharmacist team with whom professional interaction and support will be available.

Qualifications

Essential

Completion of a degree in pharmacy and registration with the General Pharmaceutical Council. Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this. Qualified independent prescriber, or working towards this. Member of the Royal Pharmaceutical Society (RPS).

Experience

Essential

Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice. Ability to demonstrate general pharmaceutical skills and knowledge in core areas. Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions. Ability to demonstrate accountability for delivering professional expertise and direct service provision as an individuals. Ability to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate. Ability to follow legal, ethical, professional and organisational policies, procedures and codes of conduct. Ability to assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance; Non-judgemental attitude and ability to be flexible and adaptable. Self-motivated with excellent communication skills.
