Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team at Norfolk Primary Care C.I.C. as a key member of our Primary Care Network in Norwich. The successful candidate will work closely with our practice team and other PCN staff to provide high-quality patient care and maintain the highest standards of medication management.

Main Responsibilities
  • Provide direct, accessible, and timely medicines expertise to patients and healthcare professionals.
  • Optimize medication issues to improve patient care, safety, and support clinical staff in the management of patients.
  • Work closely with the practice team and other PCN staff to achieve high-quality patient outcomes.
  • Act as a conduit of patient medicines information into and out of Acute General Hospitals.
About the Role

The Clinical Pharmacist will be an integral part of our General Practice team, working in one or more practices across the Primary Care Network. They will also be part of a wider pharmacist network and multidisciplinary team. The successful candidate will have the opportunity to achieve qualification from an accredited training pathway, including independent prescribing, and will receive supervision and mentorship to support their development.
