Job responsibilities

Key Responsibilities:

Patient Management:

Manage a caseload of palliative care patients, ensuring each patient receives personalized and comprehensive care. Conduct thorough assessments to determine the physical, emotional, and psychological needs of patients. Monitor and evaluate patient progress, adjusting care plans as necessary.

Care Planning:

Develop, implement, and review individualized care plans in collaboration with patients, their families, and other healthcare professionals. Ensure care plans are evidence-based and tailored to meet the unique needs of each patient. Provide education and support to patients and families regarding palliative care options and advanced care planning.

Autonomous Practice:

Work independently to deliver high-quality palliative care, making clinical decisions within the scope of practice. Utilise advanced clinical skills to manage complex symptoms and provide interventions aimed at improving quality of life.

Liaison and Coordination:

Act as a liaison between patients, families, primary care providers, specialists, and community services. Coordinate multidisciplinary team meetings to ensure cohesive and integrated care delivery. Establish and maintain effective communication channels with external stakeholders, including hospice services, social workers, and support groups.

Documentation and Reporting:

Maintain accurate and up-to-date medical records, documenting all patient interactions and care provided. Prepare and submit necessary reports and documentation in a timely manner.

Professional Development:

Stay current with best practices and advancements in palliative care. Participate in ongoing education and professional development opportunities. Mentor and support junior staff and students as required.

Quality Improvement:

Contribute to the development and implementation of quality improvement initiatives within the palliative care service. Participate in audits and research activities to enhance the quality of care provided. Person Specification

Qualifications

Essential

Bachelors degree in nursing (BSN) or equivalent

Desirable

Qualification in Palliative Care or a related field is preferred Independent Prescribing qualification desirable

Experience

Essential

Minimum of 3 years of experience in palliative care, hospice, or a related field.

Desirable

Experience working in GP Practice or primary care

Skills

Essential

Advanced clinical assessment and decision-making skills. Excellent communication and interpersonal skills. Ability to work autonomously and as part of a multidisciplinary team. Strong organizational and time-management abilities.
