About the Role

We are seeking a dedicated and compassionate Palliative Care Nurse to join our primary care team at Willows Medical Centre.

Key Responsibilities
  • Patient Management:
    • Manage a caseload of palliative care patients, ensuring each patient receives personalized and comprehensive care.
    • Conduct thorough assessments to determine the physical, emotional, and psychological needs of patients.
    • Monitor and evaluate patient progress, adjusting care plans as necessary.
  • Care Planning:
    • Develop, implement, and review individualized care plans in collaboration with patients, their families, and other healthcare professionals.
    • Ensure care plans are evidence-based and tailored to meet the unique needs of each patient.
    • Provide education and support to patients and families regarding palliative care options and advanced care planning.
  • Autonomous Practice:
    • Work independently to deliver high-quality palliative care, making clinical decisions within the scope of practice.
    • Utilise advanced clinical skills to manage complex symptoms and provide interventions aimed at improving quality of life.
  • Liaison and Coordination:
    • Act as a liaison between patients, families, primary care providers, specialists, and community services.
    • Coordinate multidisciplinary team meetings to ensure cohesive and integrated care delivery.
    • Establish and maintain effective communication channels with external stakeholders, including hospice services, social workers, and support groups.
  • Documentation and Reporting:
    • Maintain accurate and up-to-date medical records, documenting all patient interactions and care provided.
    • Prepare and submit necessary reports and documentation in a timely manner.
  • Professional Development:
    • Stay current with best practices and advancements in palliative care.
    • Participate in ongoing education and professional development opportunities.
    • Mentor and support junior staff and students as required.
  • Quality Improvement:
    • Contribute to the development and implementation of quality improvement initiatives within the palliative care service.
    • Participate in audits and research activities to enhance the quality of care provided.
