Overview

We are seeking a dedicated and organised Care Coordinator to join our team. The ideal candidate will play a vital role in managing client care plans, coordinating services, and ensuring seamless communication between clients, families, and healthcare professionals. Duties

  • Organise and maintain comprehensive care plans for clients, ensuring all needs are accurately documented and regularly updated
  • Coordinate appointments, services, and resources to meet individual client requirements
  • Act as a primary point of contact for clients, families, and healthcare providers, facilitating clear and timely communication
  • Schedule and confirm appointments, ensuring smooth operational flow within the organisation
  • Maintain detailed records of interactions, service delivery, and client progress using office management systems
  • Collaborate with multidisciplinary teams to ensure coordinated and personalised care provision
  • Assist in administrative tasks such as filing, data entry, and report generation to support team efficiency
  • Ensure compliance with organisational policies and relevant regulations at all times

Experience

  • Prior office experience is preferred; familiarity with administrative procedures will be advantageous
  • Strong organisational skills with the ability to manage multiple priorities effectively
  • Excellent communication skills, both written and verbal, with a compassionate approach towards clients
  • Proficiency in using office software such as Microsoft Office Suite or equivalent programmes
  • Ability to work independently as well as part of a team in a fast-paced environment
