An exciting opportunity has arisen for a registered health professional to work within the Personal Health Budget Team in the Hampshire CHC & Placements Team. The successful applicant will work within the PHB team to support the Continuing Healthcare service in meeting the NHS England and Improvements’ agenda that everyone living in their own home who is in receipt of NHS Continuing Healthcare funding, will have a personal health budget.

Personal Health Budgets are a way of empowering individuals to be able to exercise more control over how and when their care needs are met, by delivering NHS funds directly to them, and are an integral part of the NHS personalisation agenda and person-centred care.

This is the chance to be part of the PHB team within CHC, whose role is to support the CHC locality teams in the setting up and ongoing monitoring of PHBs. This includes providing ongoing support to PHB holders, to ensure safe and appropriate care & support is being received, and to work to ensure the NHS England targets for PHBs are being met.

**The successful applicant will**:
Act as an advocate for personalisation and personal health budgets.

Guide and support people through the process of receiving a Personal Health Budget using their budget to purchase the care and support that they require.

Work with people to develop a personalised care and support plan and use clinical expertise to support individuals to identify and commission the services that they need.

Provide on-going assistance and clinical support, where required, to support individuals in the management of their PHB

Support with training & development in relation to PHBs

Build effective relationships with clients and their families and all the multi-disciplinary team healthcare professionals involved

Liaise with external agencies to ensure proper processes are adhered to

Support with training & development in relation to PHBs

This is a specialist role and therefore we acknowledge that the successful applicant may not have any prior experience of working within Personal Health Budgets.

To support other Specialist Practitioners within the locality team with the set-up of Personal Health Budgets.

To co-produce client support plans, budget calculations, risk analysis and provide advice on direct payment regulations.

To manage own complex case load including completion of clinical and PHB reviews.

To be responsible for initiating arrangements to hand back clients that are no longer complex to the locality team.

Attend monthly PHB working group meeting to ensure we are working to the same practice across localities.

Build effective working relationships with external agencies including Direct Payment (DP) Support Contract Provider and other agencies as required.
