**An exciting opportunity**
**We are seeking a dynamic individual to work as part of a friendly team focusing on our primary aim of putting the patient at the heart of what we do. You will work as part of a multi-disciplinary team structure with a variable caseload of complex patients. You will work alongside the Hospital Discharge teams in the Single Point of Access (SPoA), medical and therapy teams, Older Persons Mental Health, health and social care community services, and GPs to ensure, every patient identified with a standalone health need and diagnosed with delirium during their acute admission, receives the most appropriate care in the most appropriate setting. A large part of this role will be around the discharge pathway.
