An exciting opportunity

We are seeking a dynamic individual to work alongside the Specialist Complex Pathways Operational Manager.

The Lead Practitioner will work as an autonomous practitioner with a variable caseload of complex patients within a multi-disciplinary team structure. They will work alongside the Hospital Discharge teams in the Single Point of Access (SPoA), medical and therapy teams, Older Persons Mental Health, health and social care community services, and GPs to ensure, every patient identified with a standalone health need and diagnosed with delirium during their acute admission, receives the most appropriate care in the most appropriate setting. A large part of this role will be around the discharge pathway.

The applicant will be a Registered Health Professional and have experience of line managing staff. The applicant must be educated to degree/working towards a degree and experience working in an acute and/ or a community based setting. Furthermore, the applicant must have an understanding of the background to and aims of current healthcare policy, NHS Continuing Healthcare and recent changes to NHS Hospital Discharge Guidance and policy.

The Specialist Complex pathway will enable patients who fall outside of the frameworks of NHS Continuing Healthcare, MHA Section117 Aftercare that have

Clinical needs which require intervention and or delegation from a registered professional,

Joint funding with the local authority (non-LD)

Or with an acute diagnosis of Delirium,

To receive the best care in the most appropriate setting for their needs.

This innovative pathway will enable patients the opportunity to be discharged from Acute hospitals whilst still experiencing symptoms. This discharge may be to their home with a tailored package of care, or to a residential setting on a temporary assessment basis. The Lead Practitioner will oversee and advocate for this process from referral onto and discharge from the pathway and ensure the team will assess, review and commission robust, safe and appropriate care packages for people (adults) referred to the pathway.
