Main duties of the job You will report directly to the Locality Lead Nurse and will be responsible for providing leadership and management to nursing and healthcare support staff within their designated area of work at Oxford Health Foundation Trust.
In collaboration with the multidisciplinary team (MDT), they will contribute to care delivery, taking a lead on specific areas.
This will be done within the available resources.As a Senior Community Learning Disability Nurse, the role involves working as part of the MDT to assess, plan, implement, and evaluate the healthcare needs of adults with learning disabilities, following the framework of: MDT collaboration principlesThe holistic care model (Four-Tier model)Seven areas of clinical practice Key responsibilities include:Managing a complex caseload of patients with learning disabilities, being accountable for conducting comprehensive nursing assessments, creating care plans, and implementing nursing interventions.
This includes developing a variety of care plans (e.g., Positive Behaviour Support, epilepsy care) and ensuring the inclusion of patients' and carers' views in these evidence-based plans.

Person specification Qualifications Essential criteria Registered Learning Disabilities Nurse or RN Adult or RN Mental Health with evidence of knowledge and experience of supporting people with learning disabilities with their health needs.
Knowledge Essential criteria Computer skills including Microsoft systems, electronic patient record systems, virtual consultations.Knowledge and experience of a variety of assessments and interventions related to Oxford Health Learning Disability Core businessKnowledge of evidenced based practice in clinical areas of practice including Epilepsy; Dementia; physical and mental health; autism; behaviours of concernWorking knowledge of the application of the NMC Code of Conduct Experience Essential criteria Experience equivalent to in depth post qualifying (at least 3 years)Experience of mentorship in practice and/or management of junior nursing staff Skills Essential criteria Ability to work autonomouslyProvide clinical supervisionEvidence of effective interpersonal skills (active listening, building empathy) All new starters have a thorough induction process, both local and Trust wide, which aims to create a positive supportive working environment allowing new employees to learn key elements of their role over a reasonable timescale.Appointment to this post is subject to the trust receiving satisfactory references covering 3 years of employment or study.
