We are looking for positive and passionate applicants for a Band 3, Healthcare Support Worker post.
This is a great opportunity to develop your skills and knowledge of working within a Specialist Learning Disability Inpatient Setting.
You will be supported by the Team to deliver high-quality, individualised care and treatment plans, ensuring we continue to maintain excellent, patient focused services in line with our national and locally agreed targets.
The Assessment and Treatment Unit provides highly specialised assessment, formulation, and intervention to meet individual needs.
The service works within positive behavioural support frameworks and provides trauma-informed care.
Learning Disability Services are undergoing a programme of modernisation and so this is an exciting time to join the service and be part of the transformation journey.
The post holder will practice as part of the multi-disciplinary team, with the expectation to use initiative and be responsible and accountable for safe, compassionate, person-centred and evidence-based care that respects and maintains dignity and human rights.

To promote and support the physical health, comfort, wellbeing and recovery of service-users, and to assist them in re-establishing their place in the community, as appropriate.
To assist Registered Professionals in assessments and care planning/ implementation and evaluation of individualized care plans.

Job description Job responsibilities 1.
Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.

Regularly reports and effectively communicates service-users health status and care aims in care records and verbal reports to Registered Health Professional staff members and other members of the multi-disciplinary team.

Relates appropriately with service-users, carer's, colleagues and others at all times maintaining professional boundaries.

At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals.
5.
Identifies, minimizes and reports risks of danger, harm and abuse to senior colleagues.
6.
At all times minimizes risks to self and others by undertaking safe working practices.
7.
At all times works within agreed and established Trust policy and operational procedures.
8.
Works with other Trust services to promote seamless service-user person centred care from referral/admission, to transfer and discharge on identified care pathways.
9.
Responsible for contribution towards planning and undertaking delivery of planned therapeutic activities with an identified group of service users.
10.
Responsible for undertaking elements of a holistic assessment, under the direction of Registered Health Professional staff.
Responsible for regularly working with service-users to negotiate and review existing plans to address identified needs under the supervision of Registered Professionals.
11.
Works in partnership with service-users to assist them in identifying activities that are meaningful to them and that support agreed health outcomes.
12.
Regularly supports service-users to access a range of activities and services to maintain their independence and promote their health and wellbeing.
