Healthcare Support Worker

An exciting opportunity has arisen a Band 3 Health Care Support Worker to join the Specialist Children’s Short Break Team at Dragon Square on a part time basis, 18 hours per week.
The service provides overnight short breaks for children and young people with severe learning disabilities and other associated complex needs such as behaviours of concern and complex health needs.
The role involves working as part of a team of Registered nurses and Health Care Support workers in conjunction with the wider multi-agency team such as school, social workers, consultants, specialist nurses, community nurses and parents/carers to meet the needs of the children and young people and achieve positive outcomes for them, as well as supporting families and minimising the risk of family breakdown.
The role of Health care support worker is very diverse and varied and involves supporting the children and young people to meet their personal hygiene needs, diet and nutrition, communication, religious, cultural needs, health and wellbeing as well as social, sensory needs and developing daily living skills and independence. There are lots of opportunities to support the children to have fun when they are on a short break.

To provide support, and spend therapeutic time with an allocated group of service users.
To promote and support the physical health, comfort, wellbeing and recovery of service-users, and to assist them in re-establishing their place in the community, as appropriate.
To assist Registered Professionals in assessments and care planning/implementation and evaluation of individualised care plans.
To provide support and day-to-day supervision to junior staff members as required, demonstrating the Health Care Support Worker role and role modelling good practice.

The postholder will undertake the following duties and responsibilities at all times demonstrating the competences assigned to the role detailed in the associated KSFoutline:
Communicates with service-users and carers, all members of the multi-disciplinary team and staff in external agencies using a variety of methods including Information Technology.
Regularly reports and effectively communicates service-users health status and care aims in care records and verbal reports to Registered Health Professional staff members and other members of the multi-disciplinary team.
Relates appropriately with service-users, carers, colleagues and others - at all times maintaining professional boundaries.
At all times ensures that own actions, support the equality, diversity, rights and responsibilities of individuals.
Identifies, minimizes and reports risks of danger, harm and abuse to senior colleagues.
At all times minimizes risks to self and others by undertaking safe working practices.
At all times works within agreed and established Trust policy and operational procedures.
Works with other Trust services to promote seamless service-user person centred care from referral/admission, to transfer and discharge on identified care pathways.
Responsible for contribution towards planning and undertaking delivery of planned therapeutic activities with an identified group of service users.
Responsible for undertaking elements of a holistic assessment, under the direction of Registered Health Professional staff. Responsible for regularly working with service-users to negotiate and review existing plans to address identified needs under the supervision of Registered Professionals.
Works in partnership with service-users to assist them in identifying activities that are meaningful to them and that support agreed health outcomes.
Regularly supports service-users to access a range of activities and services to maintain their independence and promote their health and wellbeing.
Regularly provides advice and guidance to Service-users - promoting healthy activities and lifestyle choices.
Regularly supports Registered Practitioners and Service-users during activities to meet their physical and mental wellbeing.
Regularly supports service-users in meeting their assessed personal care needs
