Community Health Nurse

We are seeking experienced Community Health Nurses to support our team in delivering high-quality care to children and young people. As a key member of our team, you will play a vital role in achieving our mission of providing the best care by the best people.
To be successful in this role, you will need to be a registered nurse with experience and/or qualifications in working with children and young people. You will also need to be motivated and enthusiastic about providing excellent care in a community setting.
As a Community Health Nurse, you will be responsible for supporting the 0-19 Team in their duties, including assisting Nursery Nurses as required. You will also be involved in undertaking interventions as directed by the Specialist Community Practitioner, such as immunization and vaccination, screening, and assessing the health needs of children and their families.

- Undertake interventions as directed by the Specialist Community Practitioner, including immunization and vaccination, screening, and assessing the health needs of children and their families.
- Liaise with other statutory and voluntary agencies to maintain good working relationships and make referrals as necessary.
- Be aware of deviations from the norm in health and behavior and intervene to protect vulnerable children by prompt action and referral.
- Assist the Specialist Community Practitioner at Child Health Clinics, monitoring the growth of babies and children, advising parents on aspects of parenting, and referring to the Specialist Community Practitioner where necessary.
- Carry out home visits as delegated by the Specialist Community Practitioner.
