Care Team Manager

We are seeking a highly skilled and compassionate Team Leader to join our multidisciplinary team at Bradley Apartments, a specialist service for individuals with complex needs. As a Team Leader, you will play a vital role in providing care and support to our service users, who may have a primary diagnosis of learning disability, mental health issue, or personality disorder.
As a Team Leader, you will be responsible for leading a team of healthcare professionals and support workers, ensuring that our service users receive high-quality care and support. You will work closely with our multidisciplinary team to develop and implement care plans, provide emotional support, and promote independence and well-being.
Key Responsibilities- Lead a team of healthcare professionals and support workers to provide high-quality care and support to service users.
- Develop and implement care plans in collaboration with the multidisciplinary team.
- Provide emotional support and promote independence and well-being of service users.
- Work closely with the multidisciplinary team to ensure seamless care delivery.
- Supervise and support staff to ensure they have the necessary skills and knowledge to provide high-quality care.
- Manage and maintain accurate records and reports.
- Hold a valid UK driving licence (preferred).
- Have strong empathy and a caring and compassionate nature.
- Be willing to support residents with personal hygiene.
- Have excellent people and communication skills, and a good sense of humour.
- Deal with challenging behaviours.
- Have a positive and professional approach.
- Be reliable, flexible, hard-working, and honest.
- Be proficient in using computer and Microsoft Excel.
- Annual salary of £27,664.
