Buckinghamshire Healthcare Community Practitioner

We are seeking an experienced Clinical Physiotherapist to join our team in Buckinghamshire. As a key member of our community healthcare team, you will be responsible for providing high-quality care to patients in their own homes and supporting their recovery.
The ideal candidate will have excellent clinical assessment and planning skills, as well as experience of working within a multi-agency/multi-disciplinary team. You will also have strong communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
This is a full-time role, with a salary range of £40,000 - £60,000 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits, including generous annual leave entitlement, pension scheme, and access to continuing professional development opportunities.
Key Responsibilities- Provide comprehensive assessments and treatment plans for patients with complex needs
- Work collaboratively with other healthcare professionals to deliver high-quality care
- Develop and maintain effective relationships with patients, carers, and other stakeholders
- Registered Allied Healthcare Professional with HCPC - Physiotherapist
- Degree/Diploma in Physiotherapy Occupational Therapy
- Experience of recent Continuous Professional Development
