We are seeking a highly skilled Physiotherapy Specialist Practitioner to join our dynamic team at Buckinghamshire Healthcare NHS Trust. This is an exciting opportunity to work with our innovative Rapid Response & Intermediate Care service, providing expert interventions and rehabilitation in the community.

Job Summary

The successful candidate will be responsible for delivering high-quality care to patients in their own homes, promoting recovery and preventing unnecessary hospitalization. As part of a multi-disciplinary team, you will work closely with other healthcare professionals to develop comprehensive care plans and coordinate delivery.

You will have a valid driving license and access to a car, as regular travel to locations throughout Buckinghamshire is required. In return, we offer a competitive salary of £42,000-£62,000 per annum, depending on experience, plus excellent benefits and opportunities for professional development.

Key Responsibilities
  • Provide triage, specialist assessment, review, case management, and evaluation of patient needs
  • Develop and implement comprehensive care plans to promote health gain and maximize independence
  • Allocate workload to members of the Rapid Response & Intermediate Care team
  • Work collaboratively with other professionals to ensure timely delivery of care
Requirements
  • Registered Allied Healthcare Professional with HCPC registration (Physiotherapist or Occupational Therapist)
  • Degree/Diploma in Physiotherapy/Occupational Therapy
  • Recent Continuous Professional Development experience
