Buckinghamshire Healthcare NHS Trust – Aylesbury, South East

We are seeking a motivated and enthusiastic individual to join our Discharge team as a Healthcare Navigator. This role offers a unique opportunity to make a positive impact on patients' lives by supporting their discharge needs.

You will work under the supervision of experienced discharge coordinators, coordinating the planning and transfer of care for patients with complex discharge requirements. Your professional development is encouraged and supported, ensuring you have the skills to excel in your role.

Please note that the successful candidate will be expected to work across various hospital sites.

  • To support the delivery of high-quality discharge and patient flow for patients with complex needs, acting as their advocate when necessary.
  • To liaise with ward staff to identify patients requiring intervention and work with colleagues to ensure all necessary assessments and referrals are made in a timely manner.
  • Communicate effectively with patients, relatives, carers, community staff, social services, and other agencies.

The estimated salary for this role is £25,000-£35,000 per annum, depending on experience.
