We are expanding our Rapid Response & Intermediate Care service across Buckinghamshire and seeking a skilled Physiotherapist to join our Aylesbury team.

About the Role

As a key member of our multi-disciplinary community team, you will provide expert interventions and rehabilitation services to patients in need. Your primary goal is to empower recovery, prevent unnecessary hospitalization, and support early discharge from hospital and end-of-life care.

Your Responsibilities
  1. Provide comprehensive specialist assessment, review, case management, and evaluation of patient needs.
  2. Develop personalized care plans to promote health gain and maximize independence.
  3. Allocate workloads to members of the Rapid Response & Intermediate Care (RRIC) team effectively.
  4. Collaborate with healthcare professionals to ensure timely and appropriate care delivery.
Requirements
  • Valid driving license and car required for traveling throughout Buckinghamshire.
  • Registered Allied Healthcare Professional with HCPC - Physiotherapist or Occupational Therapist.
  • Degree or Diploma in Physiotherapy or Occupational Therapy.
  • Recent Continuous Professional Development experience.
