Job Description

We are seeking a highly motivated and organized individual to join our Primary Care Team as a Patient Services Care Coordinator. This role will play a vital part in supporting the delivery of high-quality patient care and services across our organization.

Main Responsibilities
  • Respond to patient queries and liaise with the wider primary care team to ensure effective communication and resolution of issues.
  • Manage appointment requests and signpost patients to our range of services, ensuring timely and efficient access to care.
  • Maintain accurate and up-to-date patient records, adhering to confidentiality and data protection protocols.
  • Utilize various information systems to support efficient workflow processes and improve patient outcomes.
  • Process prescription requests in a timely manner, ensuring accurate and secure handling of sensitive information.
  • Support patient health promotion recall campaigns and encourage the uptake of our digital tools to enhance patient self-care.
  • Participate in mandatory training, annual appraisal, and continued education to maintain professional development and expertise.
  • Contribute to maintaining a safe and secure environment, adhering to health and safety protocols and confidentiality at all times.
Key Skills and Qualifications
  • Excellent communication and interpersonal skills, with the ability to engage positively with patients and colleagues.
  • Proven administrative experience, with a strong understanding of administrative processes and procedures.
  • Proficiency in using basic computer software, including Microsoft Office and email.
  • Ability to work effectively within processes and procedures, with a proactive approach to problem-solving.
  • Previous experience in the NHS is welcome, but not essential.
