About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Bucks Gateway team at Oxford Health NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing comprehensive telephone assessments and specialist advice to patients, their families, and healthcare professionals.

Key Responsibilities
  • Provide high-quality, evidence-based telephone assessments and interventions to patients with mental health needs.
  • Work collaboratively with healthcare professionals, social care partners, and community-based resources to ensure seamless care coordination.
  • Develop and maintain strong relationships with patients, families, and carers to promote recovery and well-being.
  • Contribute to the development and implementation of service improvement initiatives, ensuring that our services meet the needs of our diverse patient population.
  • Participate in multidisciplinary team meetings, case conferences, and other forums to share knowledge, expertise, and best practices.
Requirements
  • Registered qualification in nursing, social work, or occupational therapy.
  • Significant post-qualification experience in a mental health setting, working with adults with mental illness.
  • Experience of mentoring and clinical supervision of others.
  • Ability to manage a caseload and act as a care coordinator for clients on a case list.
  • Excellent communication, interpersonal, and problem-solving skills.
