Job Overview

We are seeking a highly skilled and enthusiastic Physiotherapist to join our Urgent Community Response (UCR) and Virtual Ward services. As a specialist in respiratory care, you will play a vital role in providing high-quality care to patients in their own homes.

Main Duties

As a Physiotherapist in this role, you will be responsible for providing a 2-hour response to patients experiencing a health crisis, conducting assessments and developing treatment plans for individuals on the virtual wards. You will work autonomously to perform specialist therapeutic assessments, using clinical reasoning to deliver and adapt individualized treatment programs within the patient's own home.

You will collaborate with the Hospital at Home Team, Community Respiratory Team, Community Care Teams, Northwest Ambulance Service, Social Services, and the patient's registered GP to support the patient at home. You will also work flexibly in response to service needs, ensuring the allocation of an appropriate caseload for the Therapy Assistants.
