Job Title: Advanced Learning Disability Physiotherapist

We are seeking a highly skilled and dedicated Physiotherapist to join our team in the Urgent Community Response (UCR) and Virtual Ward services at the Countess of Chester Hospital NHS Foundation Trust.

This is an exciting opportunity to work in a dynamic and pro-active team, providing specialist physiotherapy services to individuals with learning disabilities in the community. The successful candidate will have a specialist interest in respiratory care and be able to work flexibly in response to service needs.

Main Responsibilities:
  • Provide a 2-hour response for patients entering a health crisis and develop ongoing assessments and treatment plans for individuals on the virtual wards.
  • Collaborate with the Hospital at Home Team, Community Respiratory Team, Community Care Teams, Northwest Ambulance Service, Social Services, and the patient's registered GP to support the patient at home.
  • Work flexibly to ensure allocation of an appropriate caseload for the Therapy Assistants.
Requirements:
  • Degree in physiotherapy or equivalent
  • HCPC Registration
  • Membership of professional body

This is a full-time role, 37.5 hours a week, but part-time posts would be considered. The service operates from 08:00-20:00, with a base at Ellesmere Port Hospital.
