Job Overview

Working Full time: 37.5 hours per week

We are excited to advertise a Physiotherapy vacancy within our Urgent Community Response (UCR) and Virtual Ward services, which continues to develop and adapt to meet the needs of our local population. This service is provided within the Hospital at Home Service, offering many training opportunities. The successful candidate will have a specialist interest in respiratory care to manage individuals with respiratory problems in the community.

This is a 7-day service operating from 08:00-20:00, with a base at Ellesmere Port Hospital. The service provides UCR support for the residents of Chester and Cheshire West. You would be expected to cover all aspects of the shift pattern. This is a full-time role, 37.5 hours a week, but part-time posts would be considered.

You will be part of a holistic team consisting of GPs, Advanced Clinical Practitioners, Registered Nurses, Therapists, Therapy Assistants, a Pharmacist, and Clerical Support.

The service aims to provide a 2-hour response to patients experiencing a health crisis, supporting their treatment in their own home rather than requiring a hospital admission.

Main Duties of the Job

Provide a 2-hour response for patients entering a health crisis and ongoing assessments and treatment plans for individuals on the virtual wards.

Work as an autonomous practitioner to perform a specialist therapeutic assessment of patients with diverse presentations, using clinical reasoning to provide a diagnosis, deliver, and adapt an individualised treatment programme within the patient's own home.

Collaborate with the Hospital at Home Team, Community Respiratory Team, Community Care Teams, Northwest Ambulance Service, Social Services, Millbrook, and the patient's registered GP to support the patient at home.

Work flexibly in response to service needs, ensuring allocation of an appropriate caseload for the Therapy Assistants.
