Mental Health Social Worker

At Lancashire County Council, we are seeking a highly skilled and compassionate Mental Health Social Worker to join our Adult Services Mental Health Team.
We are driving forward innovative practice that will make a difference to our communities, and we need new Social Workers to help us achieve our vision of supporting people to achieve what they want to, living independently in a place they call their home.
Your priority will be to undertake and review assessments of need, risk and capacity, sourcing and commissioning care, undertaking duty tasks and supporting discharge planning while working as part of the wider multi-disciplinary team.
