About the Role

We are seeking a skilled and compassionate Senior Social Worker to join our Adult Services Mental Health Team in Lancashire. As a key member of our team, you will play a vital role in supporting individuals with mental health difficulties to achieve their goals and live independently.

Key Responsibilities
  • Undertake and review assessments of need, risk, and capacity to ensure effective support for service users.
  • Sourcing and commissioning care to meet individual needs, while working collaboratively with internal and external professionals.
  • Support discharge planning and duty tasks as part of the wider multi-disciplinary team.
