About the Role

We are seeking a skilled and compassionate Care Coordinator to join our team in delivering high-quality care packages to clients with medium to complex care needs. As a key member of our team, you will be responsible for implementing new care plans, allocating support staff, and ensuring that our service users receive the best possible care.

Key Responsibilities:
  • Implementing new care packages and allocating support staff effectively to meet the needs of our service users.
  • Basing yourself in the field and office as required, providing a consistent and high standard of care.
  • Maintaining contact with service users, either in person or over the phone, to ensure their needs are being met.
  • Ensuring person-centred care is delivered effectively, with regular updates to care plans to reflect this.
  • Meeting with clinicians and external providers to review care packages and ensure they align with company and CQC standards.
  • Preparing and sending monthly reports to the Clinical Commissioning Group (CCG).
Requirements:
  • Proven experience in a similar role, preferably in homecare or social care.
  • A relevant care qualification (Level 3 in Health and Social Care) or willingness to work towards this.
  • Excellent communication skills, attention to detail, strong organisational abilities, and competent IT usage.
  • A full UK driving licence, access to your own vehicle, and flexibility to visit clients and professionals throughout the West Midlands.
  • Commitment to sharing on-call responsibilities, including weekends on occasion.
